Seniors Outreach Services of Oxford
Seniors Mental Health and Addiction Response Team

Who: 

  • Anyone 65 years of age or older in Oxford County with a mental health concern, including cognitive impairment with responsive behaviours
  • Anyone younger than 65 years with age related cognitive changes and responsivie behaviours

Where:

  • Anywhere in Oxford County (LTCH, Hospitals, Retirement Homes, Community)

How: 

  • All referral go through the Coordinated Access 519-421-4223, after hours or high risk situations may call the CMHA Crisis Line 1-877-339-834

Why: 

  • To assist clients with responsive behaviours to remain in the community with well supported and educated staff and families while enjoying quality of life.

What: 

  • Responsive behaviours may include verbal, physicial, sexual aggression, resisting help with personal care or medications, wandering agitation

 After the Referral is made:

  • A Nurse or Social Worker will follow up on your referral to make sure we can meet your needs and set up an appointment to meet
  • A complete health and personal history will be collected from the patient, family, staff and doctor
  • A variety of cognitive functioning assessments will be completed if needed, the Nurse or Social Worker will explain all of these to you
  • We will ask our Behavioural Consultant or our Geriatric Psychiatrist Consultant to meet with you if we think it may be a benefit and if you are comfortable with their involvement

Then What:

  • Recommendations will be provided
  • Assistance will be given to implement the recommendations if need
  • Follow up will be ongoing until the situation is resolved/improved
  • Education for staff and family members will be provided in a variety of ways including at the time of the behaviour and group formation
  • Further assessment and recommendations as behaviours change
  • We work in partnership with the CCAC, CMHA, RMHCL and the Alzheimer Society of Oxford to ensure the most appropriate service and care is provided
