Tillsonburg District Memorial Hospital offers 10 complex continuing care beds to provide a comprehensive inpatient program designed for persons over the age of eighteen with complex health care needs. Our Complex Care beds are not only for residents in and around Oxford County but are available to people from all over the Southwest region.
Referrals are processed through the SW LHIN Home and Community Care and are assessed for appropriateness by the charge nurse on a case by case basis. It is a transitional care unit that cares for people of all ages with complex care needs that cannot be met in the community and or they require a longer stay in the hospital to recover before transitioning back home.
Each patient has a personalized care plan developed to meet their unique needs and the interdisciplinary care team works in collaboration with the patient to achieve these goals.
