We accept self referrals as well as referrals from your family doctor or specialist.
Referral Form
The program is free of charge as long as you have OHIP coverage. It is supported by Alexandra Hospital Ingersoll (AHI).
The Diabetes Education Program (DEP) provides both one on one appointments and group workshops on a variety of topics including:
- Blood sugar control
- Weight management
- Physical activity
- Managing your medications
- Insulin initiation and adjustment
- Foot care education
- Healthy eating
- Grocery store tours
- Stress/anxiety management
- Financial support programs
People with diabetes should take an active role in the day to day management of their diabetes.
We will help you gain the tools, skills, and confidence to best self-manage your diabetes.
Attending a Diabetes Education Program can help you self-manage your diabetes and connect with other people living with the same condition. You will have the help of a Diabetes Team who can answer your questions and provide encouragement on your journey to better health.
