- Monthly Giving

Make a lasting commitment to TDMH through a manageable monthly donation, which adds up to a significant contribution over the year. Monthly giving is convenient, automatic and cost effective. Foundation mailing and administration costs are also reduced, which helps more funds to be directed to the Hospital. By participating in our monthly giving program, your generosity will be spread throughout the year. You may make your pre-authorized donations with a credit card or through post-dated cheques. You may discontinue your gifts at any time by providing written notice to the Foundation.
- In Memory

A memorial gift can be made to remember a family member, friend or colleague who has passed away. This is a special way to commemorate the memory of a loved one. Your gift also demonstrates your commitment to improving the quality of life for those in our community. When we receive your donation, we send an acknowledgement of your thoughtfulness to the family.
- In Honour

Honouring a special individual or a special occasion with a gift to the Hospital is a thoughtful way to acknowledge someone by supporting a cause they care about. Your donation might celebrate a friendship, the birth of a child, birthday, wedding, anniversary, retirement or any other significant milestone. The Foundation will send a card to the person you are honouring with your message when we receive your gift.
- Matching Gifts

Some employers match the charitable contributions made by their employees. To find out if your company has a matching gift program, contact your employer or the Foundation office.
- Gifts-in-kind

A gift-in-kind is a non-cash gift, including securities, real estate and items for silent or live auctions. These gifts-in-kind are evaluated and appraised in order to confirm their value and to provide an income tax receipt. Not all gifts-in-kind are eligible to receive a receipt (for example, gifts certificates or gifts of services or expertise). If you have any questions, please contact the Foundation office.
- Bequest / Legacy Gifts

Bequests are vital to ensure that the Hospital is supported, now and in the future. You have the opportunity to do something extraordinary for your community and to leave a lasting impact when you include a gift to the Foundation in your estate planning.
There are many ways to leave a legacy. We encourage you to use the professional assistance of your lawyer in preparing your will and discuss the arrangements in advance with your financial advisor.
Estate planning can give you and your loved ones the peace-of-mind, but it is important to ensure that your bequest is correctly worded to avoid confusion later and ensure your wishes are carried out. - Stocks & Securities

Making a gift of stock or registered securities to TDMH Foundation can offer an attractive investment. Recent changes in the federal tax law allows greater incentives to make charitable gifts using appreciated, publicly listed stocks or securities. These significant changes have eliminated the capital gains tax when securities are donated directly to a charity. When you donate stocks or securities to the Foundation, you get a charitable tax receipt for the full value of the donated stock and eliminate the tax due on any capital gain on the securities. - Download the “Letter of Instruction” to transfer securities to the Tillsonburg District Memorial Hospital Foundation.
- Complete the sections of the form, including your contact information, the contact information for your broker and the share information.
- Contact the Tillsonburg District Memorial Hospital Foundation to assist with completion of the transaction.

When the transaction is completed a tax receipt for the amount of the donation received by the Foundation will be sent to you. Since charitable tax receipts are issued based on the closing market value on the day received in the Foundation’s brokerage account, it is important that we are notified in order to extend our gratitude and issue a timely charitable tax receipt for your gift. - Life Insurance

A life insurance policy is an excellent way to make a substantial contribution to the TDMH Foundation without affecting the security of your family. For just pennies a day, you can leave a meaningful gift to the Hospital. In the case of charitable giving, several options are available to the donor.
Donors can either:
- Sign over a paid-up policy to the Tillsonburg District Memorial Hospital Foundation and receive a tax receipt for the cash value of the policy.
- Purchase a new policy, naming the Tillsonburg District Memorial Hospital Foundation as the owner and beneficiary, and receive a tax receipt for each premium you pay. These payments can either be made directly to the insurance company or to the Foundation.
- Name the Tillsonburg District Memorial Hospital Foundation as beneficiary (not the owner) of a policy. In this case, the donor retains ownership of the policy for life. All or part of the proceeds will go to the Foundation upon the donor's demise and your estate will receive the tax savings.
All of these forms of life insurance are welcomed by the TDMH Foundation as possible alternatives to cash gifts. Like bequests, gifts of life insurance are considered planned gifts. Although the TDMH Foundation will not receive the funds until some time in the future, there is an opportunity to thank donors now for their future contribution.
- Charitable Trusts

When you set up a charitable remainder trust, you make an irrevocable gift of principal, from which you receive the net income for life. You receive a donation receipt for the present value of your gift, which may be used to offset tax on associated capital gains. In addition, this portion of your estate is not subject to probate.
- Endowments – Directing Your Donations
Creating an endowment is a wonderful way to make sure your donation makes a permanent impression on the healthcare needs of the hospital. Gifts placed in an endowment fund are invested, with the annual income used to purchase new equipment in the area of greatest need, as determined by the Hospital and the Foundation. This is a lasting way of giving to the TDMH Foundation, because the income from your investment is used year after year, while your endowment is closely monitored by the Foundation to optimize its growth. The minimum amount of money required to establish a named endowment with the TDMH Foundation is $10,000. The endowment can hold either the donor’s name, or can be named in honour of a loved one. A formal Endowment Agreement is required and may be written with the assistance of your lawyer and/or financial advisor. - Gifts of Residual Interest
This type of gift can include real estate or other assets and involves an irrevocable future gift. You retain the use of the property for life and receive a tax receipt for the present value of the gift, which may be used to offset taxes on associated capital gains. In addition, the gift is not subject to probate.
