At Alexandra Hospital Ingersoll (AHI) and Tillsonburg District Memorial Hospital (TDMH), we are here to help you during your time at the hospital. If you have any questions or concerns, we want to assist you. Whether you have a complaint, a suggestion, or something nice to say, we would love to hear your thoughts. Your feedback is important to us!

What is Patient Relations?

Patient relations is a way to keep track of how patients feel about their care. It helps to find ways to make things better so that patients and their families are happier and get what they need.

How do I share a good story or compliment?

We love to hear your stories or compliments about your time at the hospital. If you think something about your visit was really great or if you want to thank someone, please tell us! What do I do if I have a concern or suggestion for improvement of the care for my family member or that I have received at AHI or TDMH?

We want to hear what you think! If you have any questions or concerns, we encourage you to talk to the people who are taking care of you as soon as you can. The nurses and your doctor are great at answering your questions. They will listen to you and help with your concerns. If concerns are still not resolved to your satisfaction, or you are not sure who to speak to, contact Patient Relations at AHI 519-485-1700 ext. 8340, TDMH 519-842-3611, ext. 5336, or send us your comments in writing to PatientFeedback@ahi.ca or PatientFeedback@tdmh.on.ca.

We can look into, fix, and keep track of complaints. We really care about what you say, and we are always trying to make your experience better. We are available from 8 am to 4 pm Monday to Friday. We will work to respond to you within 2 business days.

If you need more support….

If you have completed a complaint process with Patient Relations and you weren't satisfied with the outcome, the Ontario Patient Ombudsman can offer free, confidential support. They help solve problems and look into Alexandra Hospital Ingersoll (AHI) & Tillsonburg District Memorial Hospital (TDMH)complaints from patients and their caregivers about the care they receive in public hospitals, long-term care homes, and Home and Community Care Services. All regulated health professionals belong to regulatory bodies called "Colleges". If your complaint is only about a regulated health professional, such as a doctor, nurse, physiotherapist, etc., you can contact the appropriate College (e.g. College of Physicians and Surgeons).
